Transcript Request

If you need to request a transcript, you can download the form below or pick up a transcript request form from the Guidance Office.

Charge: $2.00 for a hard copy and free for electronic - cash or money order only--no checks!

SPECIAL NOTE: Transcripts are available for graduates from the the last 5 years. If you need a transcript prior to the last 5 years, you will need to contact the Records Retention Department at (754) 321-3150.

To request a transcript, you will need:

Your Full Name
Date of Birth
Social Security Number & Student Number (if different from your Social Security Number)
The address of where the transcripts should be sent (home, college or university).

You may come in to drop off your transcript request or mail the request form to our school address:

Attention Registar

Students currently enrolled at Western may request a transcript before school or during your assigned lunch.
